A SIMPLE, INFORMATIVE WAY TO NOTIFY RESPONDERS OF YOUR MEDICAL CONDITION
The Medical Indicator is a free, voluntary service that allows you to identify a medical condition on your driver license or identification card.
The Medical Indicator is represented by a small icon on the front of your state-issued credential, with your medical condition listed on the back.*

PROCESS
Visit your local DMV.
Inform DMV associate that you would like to enroll in the Medical Indicator program.
Fill out the Medical Indicator Acknowledgment form at counter.
