- IRP IFTA New Application Package
Contains checklist and applications for applying for IRP & IFTA for the first time.
- IRP - A Application
Application used to make changes to Name, Address, Contact Information, DOT Number, EIN and Mailing Address. If making changes to Name, EIN DOT Number a new SS-4 must be submitted.
- IRP - B Distance
Complete when adding jurisdictions after original or renewal application Is filed. Fill in jurisdictions that you intend to travel. Use Estimated Distance Chart or IRP-F Estimated Distance Affidavit to complete.
- IRP - C Weight
Complete the allowable weight in each intended travel jurisdiction. Complete when changing existing vehicle weight. Complete when changing existing vehicle body type (i.e., from TT to TK or vise versa).
- IRP - D Units
Complete all vehicle information including the Carrier Responsible for Safety (CRFS) EIN & DOT number. Lease must be submitted when CRSF is not the applicant or account holder.
- Fleet Application
Complete when mailing address of fleet differs from account.
- IRP - F Estimated Distance Affidavit
If Estimated Distance Chart is not desirable distance; Affidavit can be completed by applicant instead. If carrier has not traveled out of Delaware for more than 18 months; carrier will be removed from IRP unless a business plan or lease stating the destination of travel is submitted.
- Estimated Distance Chart
All apportioned vehicles registered in Delaware average total actual mileage compiled each year. Carrier uses to determine jurisdictional mileage when carrier does not have actual distance upon application.
- Individual Vehicle Distance Record (IVDR)
Sample of daily distance log.
- Power of Attorney
Complete when owner is not available.
- Authorized Signatures
Complete when more than one person with company will be conducting business.
- MGVWR Certification (Manufactures Gross Vehicle Weight Rating)
If MGVWR plate is missing from vehicle; certified fabricator or dealer can complete certification.
- IRP Certification
Qualifies applicant into the Plan.
